A student must give a home and a local address at the time of admission. If either address changes while the student is enrolled, the new address must immediately be filed in the Office of the Registrar. A student is responsible for the accuracy in the address currently on file in the offices of the University.

A student wishing to change the name on University records because of marriage or divorce must complete the change of name form in the Office of the Registrar. The student must present official supporting documents for the name change. Other name changes are made upon presentation of a court order. To have a social security number changed, the student must also present a copy of the social security card.
