Enter to win prizes by getting vaccinated and filling out the vaccination entry form below. Winners will be contacted via email on Friday of each week.

How to Enter

Participants in the COVID-19 vaccination program have five chances to win a prize in the University's "Vaccinate for you, and for Others, Protect the “G "incentives program. Registering and providing proof of being fully vaccinated gives you a chance to win an assortment of prizes. More importantly, the safer our campus community becomes.

If you have not yet received your vaccine, find a provider near you. You can also call the State's vaccine hotline at 1-855-453-0774.

Eligibility

Employees and students who have completed the vaccination reporting survey will be eligible for random selection for incentives. Be advised that the COVID-19 Pfizer vaccine doses are administered 21 days apart, and the Moderna vaccine doses are administered 28 days apart.

To participate, you must:

  • Have received the Johnson & Johnson one-dose COVID-19 vaccine or two doses of the Pfizer or Moderna COVID-19 vaccines before the date of the drawing
  • Provide proof of vaccination; and
  • Registered for classes for the Fall 2021 semester if you are a student

Drawing Details

  • Participating students, faculty, and staff members must complete the vaccination entry form.
  • If you have already been vaccinated and have completed the vaccination reporting survey, you are already entered to win! If you have been vaccinated and have not completed the vaccination reporting survey– do so now for entry.
  • Winners will be asked to show their vaccination card or LA Wallet as proof of vaccination.
  • Winners will be notified via email on the day of the drawing.
  • Winners must respond to the notification email by Monday of the following drawing date.

Winners must be able to come to campus to claim their prize.

Vaccine Information

Free COVID-19 vaccines are available on campus at the T.H. Harris Auditorium in the reception area. The availability of free COVID-19 vaccinations are Monday, Wednesday, and Saturday, 8:00 a.m. to 6:00 p.m.
